Fill all details in the prescribed bio-data proforma (Annexure-I) carefully. Include:
- Full Name and Address (in Block Letters)
- Date of Birth, Date of Entry into Service, Date of Retirement
- Complete Educational Qualifications (with elective/main subjects)
- Employment History in chronological order (Office, Post held, Pay Scale, Nature of Duties)
- Nature of present employment (Permanent/Temporary/Ad-hoc)
- Details of any previous deputation
- Whether belongs to SC/ST
Step 3️⃣: Prepare Mandatory Documents
Arrange the following documents duly attested:
- Bio-data in TRIPLICATE (3 copies)
- Up-to-date Confidential Reports/Dossiers for last 5 years
- Integrity Certificate from the employer
- Vigilance Clearance Certificate
- No Penalty Certificate for preceding 10 years
- Latest salary slip (if not following Central Govt. pay scales)
- Cadre Clearance from parent department (if already on deputation)
- Educational qualification certificates (duly attested)
- Experience certificates
Step 4️⃣: Get Employer Certification
The application must be countersigned by the Employer/Cadre Controlling Authority with official seal. The certification must confirm:
- No vigilance or disciplinary case pending/contemplated
- Integrity is certified
- CR Dossier (original or attested copies) enclosed for last 5 years
- No major/minor penalty imposed during last 10 years (or list enclosed)
- Candidate will be relieved immediately if selected
